This is a simple form that sets out all information required to make your monthly mortgage payments.  It may include the following information:

  • Loan number
  • Borrower’s name and address
  • First payment due date
  • Complete breakdown of payment amount, indication the amount of the payment being applied to
    principal , interest, taxes, and hazard insurance.
  • When payment are due
  • To whom checks are to be made payable, with the address
  • An estimate of your annual real estate taxes

These items are not required but may be included:

  • Legal description of the property
  • Date of closing or settlement
  • Property value
  • Sales price
  • Loan amount
  • Interest rate
  • Term of loan
  • Seller’s name
  • Loan closer’s name
